ISO-Getting Started

To setup your own ISO relationship with ARIS Solutions you will need to fill out a series of forms. You will create either Employment Situtations Downloading the forms is a process that takes a few minutes. The Employment Situtations forms include 8 files that can each be downloaded separately. You can return to this site to download the form again if you need to. Traffic on the Internet and noise on the phone lines can slow down the process. You can save PDF files by clicking on the floppy disk icon that is in the toolbar above the forms. Wait for the forms to fully download before you try to save them.

• Generally if respite, personal care, day support, companion or recreation occurrs in the consumer's home and the care givers are not doing the same thing for other people as well the consumer is an employer and should follow the instructions for Employment Situations. Select the Download the Employment Forms from the left menu, print the forms, fill them in, sign them and send them to ARIS Solutions. These are PDF files and require Acrobat Reader. They can be saved on your computer and reprinted again if you need them.
